Recommendations

As part of the application process you need to secure the following recommendation and report forms.  In the application, you provide a name and email address, and an email invitation will be sent to your recommender, who must then submit the form online by the application deadline. You can click on the links below to preview the questions that are asked on each form.

• High School Counselor Report and Transcript.  This form must be completed online by a high school counselor who who can obtain an official transcript and who has access to SAT and ACT test results as well as school and class GPA information (preview the questions). In addition to the online form, your counselor must mail an official transcript to the Act Six office by the application deadline.
• Teacher Recommendation. This form must be completed online by a teacher who has taught you an academic subject, for example, English, foreign language, math, science, or social studies (preview the questions).
• Community Recommendation. This form must be completed online by a mentor, employer, pastor or other adult (possibly at school) familiar with your leadership potential and involvement outside of school (preview the questions).
• Pastor Recommendation (Northwest University only). Tacoma-Seattle students wishing to apply to Northwest University must must also submit a  form from a pastor or youth leader who knows you well (preview the questions).
